Requesting records from Minneapolis Lutheran High School or Concordia Academy-Bloomington

Requesting records from Minneapolis Lutheran High School or Concordia Academy-Bloomington Transcripts and other permanent records may be requested through the Concordia Academy association office. Due to privacy laws, we are only able to accept written requests.

Forms for transcripts requests may be downloaded in either Word or PDF format. The Word document is set up as a template that you can type into directly, print and then return to Concordia.

Other Notes:

Transcripts are provided at no cost to alumni or former students.

Please indicate if transcript may be faxed by checking yes or no.

Transcripts may take between 2-4 business days to process.

Forms received without handwritten signatures will be delayed.

Requests will not be honored for students with past due financial obligations.

E-mail requests will be honored only if the document is with an original signature.
